１つのフォームに複数のパスワードを設定することができますか？

Thanks for reaching out to Jotform Support. Unfortunately our Password-Protect form functionality doesn't allow to set multiple passwords, but you can only set a single password and change it whenever you like.

As a workaround, you may try to create a sort of login page at the beginning of your form, and then through conditional logic, you may show the form fields in case the password provided is correct, or keep them hidden otherwise. Something like this:
