    Hello,

    The problem is the field on the document is not the actual field on the form. It looks like, at some point , the question is removed and recreated on the form. But the field on the document refers to the removed question. Since it doesn't collect any answers, it has no data to show on the document.

    You can simply remove that outdated element from the document and re-add from the Left Panel.
