Come cambiare il logo sul mio modulo?

Avrei bisogno di sostituire questo logo in alto a sx che appare solamente quando, in anteprima, procedo dopo la pagina iniziale di benvenuto nel test:

Now let me help you with your question, you can change the logo on your form by editing the welcome page. Let me show you how:

1. In the Form Builder, click on Edit Welcome Page.
   
2. Click on the current logo image, under Welcome Image Properties, click on Remove Image, and Upload a new image.
