Avrei la necessità di creare un test con varie domande e una serie di risposte che potrebbero essere tipo, per niente soddisfatto, soddisfatto e molto soddisfatto, oppure con una scala di valori da 0 a 10 e alla fine avrei bisogno di raccogliere i dati del compilante e lui deve ricevere un punteggio finale o un profilo finale tra 3 o 4 possibili in base alle risposte date. Ho cercato tra i template ma non mi pare di avere trovato nulla adatto allo scopo.

Now, let me help you with your question. You can create a form from scratch that will contain the questions you need and turn that form into a Quiz Form. Let me show you how it's done.
1. Go to Quiz Form, then select and click Open Quiz Form option at the bottom.
- In the authorization box select Allow.
- Choose the Form in the Form Picker and select Continue.
- On the next page, choose the features you want to use in the Quiz Form.
- Scroll down to your form and choose or provide the correct answers to the questions.
- Click Save Quiz once done providing the answers.
Your Quiz Form is now ready. For more details, you can check our guide on How to turn your Form into a Quiz or Exam.
If this doesn't work for you, another option is to create your quiz form manually where you assign Calculation Values to the correct answers on your quiz form and calculate the total using the Form Calculation Widget.

Thanks for getting back to us. I have tested the Quiz App and I have not been able to replicate the issue that you are mentioning. The only time I see this error is when I turn off the cookies on my browser. If the cookies are disabled, then, unfortunately, the quiz form app will not work properly. If you are opening the app in an incognito window that has cookies turned off, then that will still cause the issue.
In addition, you can try clearing your browser's cache and cookies and then try opening the app again.

I would need this additional information, please. Once I have created the Survey Form, I would need to collect the emails of the survey participants and integrate the Jotform Form with BREVO. However, I need to create 3 lists, one for each profile resulting from the survey. What are the steps to accomplish all of this? Thank you.

Thanks for getting back to us. You can add an email element in your form to collect the email addresses of the survey participants. Let me show you how it's done:
- In the Form Builder, click on the Add Form Element.
- In the Basic tab, click or drag and drop the Email element to the form.
Jotform doesn't have a direct integration with Brevo; however, you can use a third-party application like Zapier to integrate Jotform with Brevo where you can collect emails and other subscriber contact info. You can find more details about Zapier on this page.
Unfortunately, I quite did not understand your last question 'I need to create 3 lists, one for each profile resulting from the survey'. Can you explain more so we can help you better?

From here, when participants respond to the various questions by assigning a score from 0 to 10, they will reach a total score that I would like to quantify so that the participant receives their total score at the end of the quiz. Based on the score obtained, I have identified 3 different profiles for those who have scored from 0 to 44 points, the second group from 45 to 68, and the third group above 68. Now, I need to ensure that each lead ends up in a specific list on Brevo based on their group membership so that I can send personalized emails based on the profile they obtained from the quiz. However, I don't understand how to achieve all of this. For now, I have created the quiz with the questions and the option to choose a score. In the quiz, I have also included the option to enter an email, but I can't figure out how to display the total points and how to divide the profiles into the 3 lists based on the score achieved.

Thanks for waiting. To calculate the total score, add a Form Calculation widget to the form and create a formula to get the total score for all the question fields. Let me show you how it's done:
- In the Form Builder, click on the Add Form Element.
- In the Widgets tab, click or drag and drop the Form Calculation widget to the form.
- You can rename it as you wish.
- Next, click on the wand icon and create your formula. Click on the Add Field blue button to choose the question fields.

Thanks for getting back to us. Yes, Starter Plans are only limited to 1 email and recipient. If you'd like the option to add multiple emails and recipients, you'll need to upgrade your account. For paid accounts, we suggest only 10 recipient emails per notification. You need to create a new notification if you wish to send it to more than 10 emails.

Good evening. I have activated the annual plan, but I still can't get the form to work (see screenshot). I have created 3 different emails, one for each profile based on the score obtained by users after they have completed the form. The first email is for those who scored up to 24 points. The second email is for those who scored up to 47, and the third email is for those who scored higher than 47. However, when I go to set the parameters for one email, it clears the others. How can I make sure that each user receives the corresponding email as an autoresponder based on their score? For example, if a user has a score of 20, they should receive email number 1. If another user has scored higher than 47, they should only receive email number 3. But when I try to set it up, it only takes one email setting and clears the others. What am I doing wrong? And how can I place each email in a specific list on Brevo? I can't figure out how to activate these targeted emails for each type of result. Can you help me?

It seems that I have now managed to create the triggers for the 3 emails, but I can't figure out how to link each email to Brevo, each in a different list, to activate a different email sequence for each profile. I'm not sure if I should leave the first email (in orange) as a notification or if I should delete it.
???? Do I need to leave this email, or should I delete it?

Thanks for getting back to us. The orange email is the notification email in which you will be notified every time a new submission comes in. If you do not wish to receive notification emails then you can delete them. If you want to receive a notification email after every successful submission then you can leave it as it is. With regards to the trigger emails that should link each email to Brevo, can you please share with us the link to the form you are currently working on?

Below is the link to the survey. As you can see, there are 3 final results based on the score, and I have created 3 emails using triggers (which I hope are set up correctly). I would need to link each lead that enters a profile to a corresponding list on Brevo. On Brevo, I will have 3 lists, one for each profile, where I will create a sending sequence.
