Buongiorno, oggi stiamo avendo qualche problema con questo form https://form.jotform.com/222323504661346 perché spesso non mostra correttamente uno dei campi (quello dopo e-mail e prima di nome 2). Potete aiutarmi?

Now, let me help you with your question. I've tried loading up your form and didn't bump into any issues related to Appointment field. Check my screenshot below:
I've also tried creating a new form with an appointment field to see if there'll be any issue but, as expected, everything worked the way it should. Check my screenshot below:
As you can see in my screenshot, I was able to add a new appointment field on a form without any issues. Can you give it another try and let us know what happens? If you're still having the same issue, we'll do a more in-depth investigation and work on finding a solution.
Once we hear back from you, we'll be able to move forward with a solution.
