Thanks for reaching out to Jotform Support. The error message indicates that a database query on "answersDB" failed because the client was disconnected by the server due to inactivity. To fix this, you need to adjust the server's timeout settings or find out why the client was inactive.

I cloned one of your forms to see if I could replicate the issue but everything was working properly. I was able to submit the clone version of your form without getting an error on query message. Check out my screencast to see my results:

If your form is embedded on a webpage, please share the link to the website as well so that we can check it. I have also cleared your form's caches. In the meantime, can you give it another try and let us know what happens? Also, it would help us to understand what’s happening if you could share a screenshot of your browser's Console Log when the error happens again.
