Modulo da cui far mandare un messaggio

Buongiorno,

vorrei sapere se ci sono moduli o app che permettono di inserire un numero di telefono e una email sui cui inviare un messaggio predefinito.

Sarei grato se si potesse avere una modello da valutare.

Vorremmo un modulo in cui un utente che accede al nostro sito potrà inserire un nome e cognome, una email e un numero di telefono e cliccare su un tasto "invia promemoria". Dopo l'invio, il numero o la email che è stata indicata riceverà un sms o una email con un messaggio di testo, ovvero un promemoria.

Thanks for patiently waiting. I checked our Contact Form Templates and quickly found some examples of templates you might find suitable. I picked three that require no editing, because they include all the fields you need but no other fields. There are over 400 Contact Form Templates on Jotform, so it's quite easy to find the type you like.

The Lime Theme Contact Us Template and Blue Sky Contact Form are classic forms, and the Modern Contact Form is from the card form templates. They show one question at a time instead of all on one page.

After you've clicked the Use Template button, you only need to add an Autoresponder Email to send the message you want to the form fillers. It's simple to do. Let me walk you through it:

1. In Form Builder, click on Settings in the orange navigation bar at the top.
   
2. Click on Emails in the menu on the left.
   
3. Click on Add Email and then on Autoresponder Email.
   
4. Edit the content to your liking and Click on Save in the bottom-right corner to save your email.

You'll find our full guide about setting up an Autoresponder here. The Autoresponder will automatically pick the right recipient email, which is whatever the user filled in the email field. So no settings changes are needed, but this guide explains how to use all the settings, if you decide to customize your email with them.
