Hi Paul,
Thanks for reaching out to Jotform Support. I cloned your form and tested it to see if I could replicate the issue, but everything was working properly. Check out the screencast below to see my results:
Note that if the appointment type is set to one-on-one, it means that each time slot is reserved for only one person. To prevent the same person from booking multiple slots, you can set up a unique field. Let me show you how:
- In Form Builder, in the orange navigation bar at the top of the page, click on Settings and then click on Show More Options.
- In the Form Settings menu, scroll down and select Email in the Unique Field dropdown menu.
Currently, the Unique Field feature is available only for the following elements:
- Short Text Entry
- Dropdown
- Number
