Now, let me help you with your question. I went ahead and checked your form and can see that you're using a Card layout while the second form uses the Card layout, which is why there's an option to add a Welcome page.

You can either switch the form to add a Welcome page, or you can just add a Header element to your form and insert the logo above the form. Here's how:

1. In Form Builder, click the Add Form Element.
2. Under the basic tab, search and add the Header to your form.

Then add the logo in the Add Your Logo button at the top of the form.
