Now, let me help you with your question. I understand that you want the Name, Surname and Tax code to appear in uppercase. When I cloned and tested your form, everything worked the way it was supposed to. Take a look at the screenshot below to see my results: 

Or were you perhaps referring to the Field Labels or Sublabels? If so, you can edit and type them in uppercase. Check the screencast below:

• Click the Field you want to edit.
• Click the Gear icon to see Properties.
• Edit Field Label and Sublabels and type the text in uppercase.

Give it a try and let us know how it goes. But if this is not what you're looking for, let us know, and we'll do some more testing to see what's going on.
