Tabella due colonne

Buongiorno, sto provando a realizzare una tabella con due colonne in cui nella parte sinistra riporto un testo di default, mentre nella colonna di destra riporterei un "radio button".
L'utente può scegliere solo un'opzione di quelle elencate.
Ho provato a fare diversi test, ma non riesco.
Potete aiutarmi?

With that being said, you can take advantage of the input table to achieve what you're trying to do. Let me walk you through it:

• In the Form Builder, add the Input table element.
• Edit the input table according to your needs.

By default, once you add the input table it's already using the radio-button input type. You can just remove the other columns until there's only 2 left, the column for the row headers and the column where the user will select their choices.

If that's not exactly what you're trying to do, let us know and provide more details so we can assist you better.

Avrei bisogno:
- nella colonna di sinista "Descrittori" avere i campi di testo di default, uno per ogni riga
- nella colonna di destra"Livelli" avere un pulsante "radio button" diverso per ogni riga.
ogni pulsante ha un significato particolare

Users can only select one option per row.
