Creazione di un modulo a risposte multiple

Salve ho bisogno di creare un modulo dove ad una domanda, oltre a selezionare la risposta (che sono in genere 4,5 opsioni), per ogni risposta si possa selezionare "la quantità" tipo "raramente", "spesso" "mai". ecc...come posso fare
