Now getting back to your inquiry, as what I have understood, you want to know if it's possible to create a form with conditions, right? If yes, you can certainly do that in Jotform. Using Jotform's Conditions feature, you can create a form with conditions. Let me show you how:

1. In the Form Builder, navigate to Settings at the orange navigation bar.
2. Click the Conditions on the left.
3. If you want to show/hide a certain field in your form, click on Show/Hide Field option.
4. In the If field, select the field that you want to trigger the condition.
5. Then, select a field State and Value.
6. In the Do field, select an action.
7. Now, select the field you want to show/hide.
8. Then, click on Save, and you're done.
