"Nilai Tidak Lengkap"

Hari ini saya mendapatkan masalah saat akan submit form, stiap selesai halaman terkahir muncul notifikasi error dengan keterangan Nilai Tidak Lengkap (screenshot terlampir)

sehubungan dengan pentingnya form ini mohon segera di infokan apa solusi apa yang bisa diupayakan.

So it seems the problem is not from my form? please let me know if there's anything I have to do to fix this problem.
