    Hi Tia,

    Thanks for contacting Jotform Support. Your account was suspended by our system because your form notification was caught by our phishing detector containing restricted keywords. These keywords were commonly used by spammers/phishers to send fraud emails using our system. We have to suspend accounts doing this without exception.. I have manually reviewing your form, I don't see anything malicious in it so I lifted the suspension. I have also added your form notification into our whitelist so hopefully, this won't happen again.
