Kenapa akun saya di banned ?padahal saya tidak ada melakukan kesalahan.
Bagaimana saya bisa memulihkan akun saya kembali.

Hi Muhamad,
Thanks for using Jotform. When I checked your account, I saw that it was auto-suspended by our system because your form titled Formulir includes fields to collect sensitive information, which violates our Terms of Use.
I’ve removed the offending fields, and reactivated your account.
