I checked a few of your Smart PDF forms and see that they seem to work as expected. Please note that the system tries it best to detect the fields from your PDF and automatically create them for you. However, due to the nature of PDF files, some fields may not be created. In this case, you'll have to create those fields, and map them to your Smart PDF form. Let me show you how to do that:

1. In Form Builder, click on the Add Element button on the left and select the field of your choice.
2. Next, click on your field, and map it to your PDF on the right.
