Thank you for reaching out to us for help. our form is currently disabled which is why you are getting the error message This form is currently unavailable. Enabling the form again is easy let me show you how:
- Open your form in Form Builder and in the orange navigation bar click on Settings.
- Click on Form Status, and change the status from Disabled to Enabled.
Once the form is enabled, they should be able to access your form without receiving the error message. We also have a guide that you can check out here on how to disable and enable forms.
Give it a try and see how it goes.

Now, let me help you with your question. I've checked your DGO East Java & Balinusra - Registrasi Mitra Dulux form and can confirm that it is enabled. I've cloned the form to test it and I was not able to replicate the issue. I was able to submit the form.
I've cleared your form cache. Can you please test your form to see if it makes any difference? If the issue persists, will you give us permission to do a test submission on your form?
