iMessage is an Apple product. The iMessage Support section of Apple’s comprehensive website doesn’t mention HIPAA or Business Associate Agreements. Those features are only mentioned for the Apple Health App.
Users who need to use a messaging app for PHI may want to consider the Apple Health App or a different app, rather than iMessage.
