Bonjour,
Merci.

Hi Nicolas,
Thanks for reaching out to Jotform Support. Unfortunately, our French Support agents are busy helping other Jotform users at the moment. I'll try to help you in English using Google Translate, but you can reply in whichever language you feel comfortable using. Or, if you'd rather have support in French, let us know and we can have them do that. But, keep in mind that you'd have to wait until they're available again.
Now, let me help you with your question. We don't provide custom form creation or designing services, but you're welcome to work with a third party who may be able to help you. See our Jotform Solutions Partner Directory for a list of consultants who have asked to be included in our directory.
You can get in touch with the right Solutions Partner for you by sorting them by location, type of service, industry, language support or partner tier. Please fill out the contact form of your preferred Jotform partner so that they can make necessary preparations for your inquiry.
Please note, however, that these parties operate independently from Jotform, so we assume no responsibility or liability for their work or their actions. For your next question regarding your form. You may use Calculation Values and pass it to a Short Text element to show it to your Table. Let me help you how to do that:
- Open your form in Form Builder. Click on your Input Table and click the Gear Icon.
- Click the Options tab and type in the values that you want to show in your table.
- Click on the Add Form Element menu on the left side of the page.
- Under the Widgets tab, drag and drop a Form Calculation Widget to your form.
- Copy this calculation below:
- That's it.
This is how it will look like after adding calculation values:

Give it a try and let us know how it goes.

Hi Nicolas,
Alternatively, you can also use Multiple choice for your field. The selection will show per item in the submissions table. Let me show you how to do that:
- In Form Builder, click on the Add Form Element menu on the left side of the page.
- Under the Basic tab, drag and drop a Multiple Choice element to your form.
- Click the Gear Icon, and go to Options tab. On Options section, type/paste your items. That's it.
This is how the Table will look like when a submission is made:
Give it a try and let us know how it goes.
