Bonjour,

J'aimerais créer un Jotform pour faire signer une décharge de responsabilité.

Ainsi, j'aimerais que le client puisse inscrire son numéro de membre et que ses informations s'affichent (prénom, nom, couleur de bracelet, type de membre, etc).

J'ai tenté la saisie automatique, mais cela fait que le client pourrait modifier ses informations, ce que je ne veux pas.

Est-ce que cela est possible?

Now, let me help you with your question. We have a Spreadsheet to form widget widget that you can use to import your data, but it won't populate the Product List field. Let me show you how to use it:

• Create a spreadsheet that includes your data in the structure you see below. It's important that the column names in the spreadsheet and field names on the form are the same.

• In Form Builder, click on the Add Form Element menu on the left side of the page.
• Under the Widgets tab, search for the Spreadsheet to Form widget and add it to your form.

• In the Widget Settings window that opens up, upload your data file and customize the other options. 
• Then, click on Update Widget.

• In the final stage, add the fields to view values and name them the same as on the spreadsheet.

Merci de revenir vers nous. Si vous souhaitez empêcher le répondant de changer l'un des champs du formulaire, vous pouvez régler ce champ en Lecture seule. C'est facile, laissez-moi vous montrer comment.

1. Dans le Générateur de formulaire, allez à la page Créer de la barre principale orange.
2. Sélectionne le champ à régler et cliquez sur l'Engrenage.
3. Dans l'onglet des Propriétés du champ, ici un Texte court, cliquez sur l'onglet Avancés.
4. Réglez sUr On l'option Lecture seule.

Et voilà.

Dans cet exemple simple où le champ Value 1 est automatiquement rempli à l'aide du champ Code Unique relié à une feuille de calcul avec le widget Spreadsheet to form, ce champ Value 1 peut bien être sélectionné, mais pas changé.
