Duplicate fields are showing in the form

Bonjour, je vous contact car dans la page creation ou je met mes questions... tout est réaranger dans l'ordre mais lorsque je met "previsualiser le modele" ou meme "envoyer le questionnaire et que je louvre pour le remplir sur une nouvelle page" , il y les questions du debuts qui apparaissent juste en dessous de "soumettre" , ce qui donne une repetition et un probleme d'affichage

Now, let me help you with your question. I checked your form, and it looks like you've duplicated several fields in your form. If that's not what you intend to do, simply delete the duplicate fields. Then rearrange the fields to your requirement. If the duplicate fields still show up after deleting them, let us know the name of the field, so we can check it.
