Now, let me help you with your question. You can easily create a form via the My Forms Page, let me show you how to do it;

• Go to the My Forms Page, then click on Create Form.
• You can use our 10000+ Templates by clicking on Use Template.
• Or you can click on Start from Scratch and choose your form Layout "Classic or Card".
• Click on Add Form Element and start creating your form by selecting its elements.
