Is this only happening in your inbox? Can you check the submissions table to see if the answers were showing? I can also see that you have a Notification email set up in your form. Can you check that if there's any information showing? See the screenshot below:

If there's no information showing, it could be that the form was submitted without filling in the fields. I've cleared your form caches to refresh the data.
