Bug du formulaire - Erreur 404

Bonjour,

Nous remarquons que le formulaire bug sur un ordinateur (voir printscreen ci-dessous).

Nous avons remarqué le même problème avec d'autres formulaires Jotform, mais il n'y a pas de soucis sur mobile. Pouvez-vous nous informer d'où vient le problème et y remédier?

Now, let me help you with your question. I tried to access the form on my end and it was working properly. Check out the screenshot below:

However, it turns out that European internet service providers have unfortunately blocked access to the subdomain https://form.jotform.com. We are already in contact with these internet service providers to restore the proper functioning of this link as quickly as possible.

In the meantime, you can change the links to your forms that start with: https://form.jotform.com/formID to https://eu.jotform.com/formID

You can also ask your internet service provider to remove the form.jotform.com domain from their blacklist.
