Thank you for your patience. The account has been unlocked. However, since your account is HIPAA-compliant and requires extra security measures, it is strongly advised that you wait for an hour to avoid any possibility of it being locked.

Hi Marilyne,
I am the Enterprise account manager for CISSS des Laurentides.
We received multiple new requests on our Enterprise Sales contact page to unlock your account. As I mentioned in the email yesterday, group email aliases are not allowed as it is a violation of our terms as it goes against our Terms of Use and also it is not allowed on HIPAA-compliant servers at all. By sharing this account with other users, it is leading to this user ID being linked with this email address being automatically locked out by our system.
Please consult your administrator if you require to be added to your Enterprise Jotform account, as you would need to become a paid user with your own login tied to a unique email address to access the platform.
