Bonjour,

Hi Guillaume,
Thanks for reaching out to Jotform Support. Unfortunately, our French Support agents are busy helping other Jotform users at the moment. I'll try to help you in English using Google Translate, but you can reply in whichever language you feel comfortable using. Or, if you'd rather have support in French, let us know and we can have them do that. But, keep in mind that you'd have to wait until they're available again.
Now, let me help you with your question. If you are using the Card Form, the Emoji Slider is available and you can set its Input Type to the Evaluation Slider. Here's how to do it:
- In Form Builder, on the left side of the screen, click on Add Form Element.
- Scroll down to Emoji Slider and click/drag to add to the form.
- On the right side of the Emoji Slider, click on the Gear icon.
- On the right panel of the screen, click on Fields and select Evaluations Slider under Input Type.
For Classic Form, you might want to check out the Numeric Slider and Slider widgets. Here's how to do it:
- In Form Builder, on the left side of the screen, click on Add Form Element.
- Go to Widgets and search for Numeric Slider or Slider.
- Click/Drag the widget to add to the form.
Give it a try and let us know how it goes.

Hi Christopher,
Thank you for your reply.
I tried the "slider" and "numeric slider" widgets. Unfortunately, unless I'm mistaken, we cannot enter a custom field in values like "€9/month". I want the slider to be able to enter something like "€3000 insured for €12/month".
I can't find "emoji slider" but I don't think it allows the functionality I want

Hello Guillaume,
Thanks for reaching out to Jotform Support. I understand that you would like to use a slider to show the amount based on the monthly insurance, is that right? Please check the screencast below if it fits your requirements:
If so, let me show you how to use the Slider with Calculated Result widget:
- On the Form Builder, under the Build tab.
- Select the Widgets tab on the Form Elements panel.
- Add the Slider with Calculated Result widget.
- Click the Wand icon, and select the General tab.
- Under the Configuration section, input the details for the slider values.
- Scroll down, and under the Calculation Formula, input the calculation of the monthly insurance based on the selected Coverage.
- Then, under the Result Text, this will be the text that will show based on the calculation.
Also, if you are looking for the Emoji Slider, it is only available on the Card Form Layout.
Let us know if you have any other questions.
