Regarding the missing values from the form fields. I have checked your form but did not find a field with this name: Remplissez les champs vides. Are you referring to this form or what? Can we make a test submission on your form, to test it and see how it works, and if there are any missing values? Could you please provide the exact steps in order to reproduce the same error?

For the Chiffre field, are you referring to this one?
