Review a Form

Bonjour nous somme l'AQDR Outaouais un organisme à but non lucratif dont la mission exclusive est la défense collective, la protection et la promotion des droits des personnes ainées, retraitées et préretraitées. Au mois de février nous avions eu à faire une demande pour le fond de relance des services communautaires Centraide-Outaouais en février 2022 et aujourd'hui nous avons besoin de revoir le formulaire que vous avez sauvegardé pour nous mais impossible de le voir.

I have checked your account and did not find any forms under it. Regarding your screenshot, could you please copy this form URL, and send it here in order to check? Also, I did not understand how do you want us to review it. Do you think it is a suspicious spam form, and it should be disabled or what?

Thanks for getting back to us. I have checked this form, but I can see that it is set as Private. I can also see that this form is not associated with your account email address. Could you please elaborate more on how can we help you? Are you the form creator, but can not access to your account or what?
