How to show another form based on the user's answer?

With that being said, let me help you with your issue. If you want to redirect your form fillers to another form based on their answers on your form, you can do it with the Change “Thank You” Page conditional logic. Let me show you how:

1. In Form Builder, in the orange navigation bar at the top of the screen, click on Settings.
2. In the menu on the left side, click on Conditions.
3.  Select the Change “Thank You” Page option.
   
   
4. Set the conditions like you see below:
   
5. Add another condition and set up conditions like you see below:
   
   
6. Once done, click on Save to save the changes.

In my demo form, if the answer to the question is Yes, the form fillers will be redirected to Form 2, if the answer is No, they will see Form 3. You can clone my demo form to see the complete setup.
