Formulaire en 2 étapes

Bonjour,

Dans le cadre de nos évaluation annuelle de performance, j'aimerais créer un formulaire avec 2 parties distinctes:

• 1 premiere partie a remplir par l employé(e)
• et une 2e partie qui sera rempli par son manageur pendant un entretien individuel qui aura lieu après que l'employé(e) ait soumis son formulaire

Cela est il réalisable?

With that being said, let me assist you with your request.  Yes, you can have multiple users fill out the form. Simply add the Edit Link on the Notification or Autoresponder Email sent to the Admin after the Employee filled out the form. The Supervisor will have to access the form using the Edit Link to continue filling it out. Let me show you how to add the edit link:

• Open your form in Form Builder, and in the orange navigation bar at the top of the page, click on Settings.
• In the Settings menu, click on Emails and hover your mouse over the Notification or Autoresponder Email.
• Click on the Pencil icon, and in the window that opens, click on the Form Fields dropdown menu and select Edit Link.
• After adding the link, click on Save, and you're done.

After submitting the form, the submitters will get an email like the one below:
