Now, let me help you with your question.  Is this happening only to 1 user? Is your form embedded on a website? If embedded, please share the link where your form is embedded.

In the meantime, I cleared the cache on your forms. Can you try again? If the issue persists, will you allow us to do a test submission on your form?
