As for your question, I checked the form, and it appears that you have set the Form Status option to Disable on a specific date. Check out my screenshot below to see my result:
To fix this, you need to change the status of your form to Enabled. Here's how to change it:
- Click Settings in Form Builder.
- In the Form Settings, set the Form Status to Enabled.
