Now, I'll help you with your question. I tested the form on a clone and was able to replicate the issue. Please make sure that the unique name matches for each field. See example below for the name and date. The Unique name for Name should be added as {nom} and for the date {dateDe125}.

Please check the correctness of each unique field to make sure they match and try it again.
