How to Change the Page Title of the Form

J'ai pris un formulaire comme modèle... et quand je l'envoie aux personnes ceci indique l'ancien nom pour compléter leur formulaire - le nom qui apparait en haut est Inscription municipale de musique

mais le nom qui devrait paraître est Membriété de Pluri-elles - mai 2023
