Now, coming back to your question, are you looking to hide the fields based on the data in a spreadsheet? It might be possible using the Spreadsheet to Form widget and the conditions. You can also autocomplete the Spreadsheet to Form widget using the URL parameters as explained here. If this is not what you are looking for please explain more and we'll do a more in-depth investigation and work on finding a solution.
