En préchargeant des données de mon application dans le formulaire.

Now, coming back to your question, do you mean to populate the form fields with data submitted from your PHP application? If you have the programming resource, you can create a custom program using a server side script, where in your case is PHP, to create an interface or connection to pull data from your database to the form fields. You will also need to use the form's full source code then.

We don't have a specific guide about of doing it, but you may also consider hiring another developer to do this, or you can consult with any of our Jotform Solutions Partner.
