Now, coming back to your question, your account was suspended because you’ve got a form named FedEx.com that’s breaking our Terms of Use by collecting credit card information in plain text fields and copyrighted logo.

In line with our PCI certification, we have to deny the collection of credit card details on regular forms. But, we have other PCI-compliant ways you can do this. All you need to do is choose one of the payment processors available on our platform and integrate it into your form. You still won't be able to collect credit card details directly, though. If you only want to collect your customer's payment details and charge them at a later date, you can take advantage of our Payment Authorization feature. 

Also, it appears that your form was flagged as having a copyrighted logo. If you believe that your account was suspended in error, we’ll need you to do the following:

• Send us the URL where your form is used.
• Send us a link to your website or proof of affiliation with a legitimate institution.
• Change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

Send these things above to reviewers@jotform.com with Thread# 15238253 as the subject, and we’ll let you know the status of your account after we’ve done a thorough review of them.
