How to add image as a Single Choice?

Now, coming back to your question, you can use the Image Radio Buttons to use images as a single choice question. Here's how to do it:

1. In Form Builder, on the left side of the screen, click on Add Form Element.

2. On the left panel, go to Widgets.

3. Search for Image Radio Buttons.

4. Click/Drag to add Image Radio Buttons into the form.
