Now, coming back to your question, As I checked, your account has been suspended for a possible scam and collecting credit card details which is against our terms of use. We need to delete it before we can reactivate your account. If, however, you are a legitimate institution and don't want us to delete the flagged form, then we need you to provide the following information.
- The URL where their form is used.
- Any document that will support your claims.
- Your website or proof of affiliation to legitimate institutions.
Please send these proofs to reviewers@jotform.com with Thread #14248401 as your subject. We will let you know about the status of your account after a thorough review of your proofs. We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.
