Bonjour j’aimerais récupérer mon accès à mon compte associé à mon site web actioncr.ca car j’aimerais ajouter l’obligation au client d’entrer certaines données avant de pouvoir envoyer un message ou une demande de soumission.

Now, coming back to your question, To clarify, do you want to gain access to the account with the appointment request form? May I know if you have access to the email address mic****l.viz****@hotmail.ca? If yes, you can try using the reset password to gain access to the account. You can follow this guide on how to reset the password.
