Preview Upload Image is not working

Now, coming back to your question, I have tested the Image Upload Preview on your form and I didn't encounter any issues as shown in the screen recording.

Did you encounter an error message when uploading an image? It would help us to understand what’s happening if you could take a screenshot and send it to us. We have a guide here explaining how to do that.
