Unfortunately, collecting login credentials is a violation of our Terms of Use. Any account collecting login credentials will be suspended automatically. Since there's no submission in the form, I've gone ahead and deleted the offending field, and reactivated your account. Please read the terms of use to prevent future suspension of your account.

Thanks for getting back to us. You can ask for codes in the form. But collecting login credentials is prohibited and may result in suspension of the form and the account.
