Now, let me help you with your question. I checked your account and found that it's been suspended because this form 232745091613050 is collecting sensitive information. Collecting sensitive information, like SSN, and driver's license numbers is against Jotform's Terms of Use.

For us to activate your account, you'll need to give us permission to delete the offending fields from your form. Would you like us to move forward with deleting the offending fields?

After we hear back from you, we'll be able to move forward with this.
