Clarification de la tarification de Jotform pour les questionnaires médicaux

Bonjour,

Je souhaite dématérialiser la complétude d'un questionnaire médical que je joins en pièce jointe où il est demandé la signature du patient. Environ 150 patients par jour.

Je n'ai pas bien compris les tarifs et quels modules je dois acheter pour se faire ?
