Now, let me help you with your question. Are you referring to the Réservation pension form? I cloned your form and tested it to see if I could replicate the issue, but everything was working properly. I was able to submit the form without any issues. Check out the screenshot below to see my results:
I've cleared your form caches to see if that will fix the issue. Please clear your browser cookies and cache, and then try submitting the form again. If the same thing happens again, can you share a screenshot of the console log when the issue occurs?
Also, for us to investigate it more, do you allow us to try making a submission on your form?
Once we hear back from you, we'll be able to move forward with a solution.
