I'll try to help you in English using Google Translate, but you can reply in whichever language you feel comfortable using. Or, if you'd rather have support in French, let us know and we can have them do that. But, keep in mind that you'd have to wait until they're available again.
Now, let me help you with your question. I checked your account associated with this email address: t.gaillard@servettefc.ch, and it appears that it is suspended due to asking for credit card information, which is against our terms of use. In order to activate your account, we need your permission to delete this offensive field from your form.
Once we hear back from you, we'll be able to help you with this.

Je n'ai pas connaissance de cette demande dans l'un de nos formulaire. Toutefois, je vous confirme que vous pouvez annuler celui-ci afin que nous puissions de nouveau avoir accès au Jottform et effectuer le paiement de notre abonnement Premium.
Pouvez-vous pour information me donner le nom du Jottform afin que nous puissions voir en interne qui a crée celui-ci ?

Hi fviretto,
Thanks for getting back to us. The email address my colleague provided earlier is what's associated to the account. As for the name, it appears to be using a Company name, nothing specific to a certain someone's personal name. The name on the account is Hospitalité Servette FC.
If you happen to have access to the email address, you can reset the account's password in order to gain access to it. However, you can't do that while the account is suspended. As mentioned earlier, we will need to delete the offending field in the form in order to reactivate the account.
Once we hear back from you, we'll be able to help you with this.

Hi Florian,
Thanks for getting back to us. I've reactivated your account. You can now proceed in resetting the password. Let me walk you through it:
1. Go to the Jotform Main Page and click on Login.
2. On the login screen, click on Forgot Password.
3. Enter your email address and then click on Send Reset Instructions.
4. You'll get an email with instructions about how to reset your password. In the email, click on Reset Password.
5. Enter your new password, enter it again to verify it, and then click on Create a New Password.
Your new password is now set and you can use it to log into your Jotform account.
