Account Suspended: Collection of login credentials

Or, if you'd rather have support in French, let us know and we can have them do that. But, keep in mind that you'd have to wait until they're available again.

    Now, let me help you with your question. I've checked your account and found that it was suspended because your "Questionnaire" form is collecting login credentials. Keep in mind that collecting login credentials is against Jotform's Terms of Use. To reactivate your account, can you permit us to either delete the form or the offending fields?

    Once we hear back from you, we'll be able to move forward with this.
