Now, let me help you with your question. The Section Visibility and the Button Visibility for the 2 Section Collapse elements at the top of your form are all set to hidden. In this case, it will not display any form fields below it. The Section Visibility will display the entire section and the Button Visibility will display the button only. At least one of these should be set to Visible in the Section Collapse Properties. To go to the Properties, select the Section Collapse element and then click on the gear icon on its right.
