Now, let me help you with your question. I checked your account and I see that there are forms added in the two Teams you created. Can you please check if those are the missing forms you're looking for? If those are the forms, and you want to move them back to your My Forms page, let me show you how to do it:

1. Click on the Team on the left side of the page.
2. Select the form/s,
3. Click on Move to My Forms button on top of the page.
